HOW IT WORKS

The All-On-4 procedure involves removing your remaining teeth, placing 4-6 implants per arch, and securing a low-profile bridge to the implants. The implants are often placed at specialized angles to avoid the patient’s sinuses. This also avoids the need for expensive bone grafts that come with long healing times.

The traditional All-On-4 procedure involves 4 implants and acrylic denture teeth over a titanium frame. Our doctor has modified this procedure to provide an even better result for his patients based on his experience successfully placing countless dental implants. At Syringa Dental & Implants, we carefully analyze each and every case and base our treatment on your individual anatomy. Your doctor will place between 4 and 6 implants per arch and temporary fixed bridges will be secured while your implants integrate with your jawbones and your mouth heals.

After your healing is complete, the doctor and lab technician will design your final All-On-4 teeth with zirconia, which is a premium material that’s much stronger than the traditional acrylic (plastic) denture teeth offered at most other dental offices. Zirconia gives a more natural, lifelike result which is also more hygienic and resistant to staining. This means your teeth will last longer and look better than the acrylic/titanium hybrid bridges you’ll find with many other All-On-4 specialists.

AM I A CANDIDATE?

The majority of patients, including those who have been previously turned away due to insufficient bone for dental implants, are suitable candidates for the All-On-4 procedure. The Dental Warranty is for six years through the Dental Warranty Corporation and is purchased at time of service. Silver package is not eligible for warranty. All-on-Four Maintenance: To maintain your oral health and the integrity of your dental implants, six month cleanings of the prosthesis along with a complete evaluation of the dental implants is necessary. The cost for this service is $300 and may not be covered by dental insurance. The first maintenance appointment will be scheduled six months after the delivery of the final prosthesis and is included in the price of the package.

Dr. Loftus is an expert in all-on-four implant dentures. He received his doctorate from one of the top clinical programs in the country where he received awards for Dental Implants. He is recognized by the American Academy of Implant Dentistry as an Associate Fellow due to the care that he’s given his patients. Dr. Loftus has completed implant continuums specific to treating patients in need of all-on-four implant dentures.

Frequently asked questions

Will I be able to eat with my new teeth from day one?

Yes! However, we will have you hold off on taking a big bite out of an apple or corn on the cob until after your healing period (3 months). The reason for maintaining a diet that consists of softer foods is that the bone around your new implants takes up to 3 months to heal. A general rule is to stick to foods that you can cut up with a plastic fork and knife. If it’s too tough for that, then it is likely too tough for your newly healing bone. But once we get to your final teeth, bring on the steak!

How long has the All-on-4 procedure been around?

The first implant denture was done in 1965 by Dr. P Branemark and lasted until the patient’s death of natural causes in 2006! Implant materials, technology, and techniques have improved and advanced since then. The hope and expectation is that each All-on-4 that is done at Syringa Dental and Implants lasts for the rest of each patient’s life.

What will my final teeth be made of?

We make the final bridge out of zirconia. Zirconia is much stronger and more resistant to staining and wear than traditional acrylic All-On-4 teeth that many other offices provide. That is a good thing to clarify when investigating other offices. Acrylic, or PMMA, breaks down, stains, and can fracture at much more accelerated rate and is generally thought of as a temporary material. 

How do I care for my All-On-4 teeth?

Caring for your teeth is very easy, but consistency is key. Caring for your teeth consists of:

  • Daily brushing of your zirconia teeth.
  • Daily cleaning of under your zirconia teeth (even more important than cleaning the teeth) to keep your gums around your implants healthy. 
  • Regular Dental Cleanings with your hygienists every 6 months to 1 year. 
  • Protect them from daily habits (like night time grinding) or contact sports by use of an occlusal guard.

How much does the All-On-4, “Teeth in a Day” procedure cost?

The total cost for a single arch (upper or lower) is $20,000 for zirconia teeth and $15,000 for a long term temporary set of teeth that can be upgraded to zirconia. For both arches, the total cost is $39,500.  These prices include removing any remaining teeth, placing your implants, same day fixed temporary teeth, and your fixed permanent zirconia teeth. Many offices will charge a surgery fee, temporary fee, and then surprise you at the end with another fee for your final teeth. Our pricing is completely transparent, and we will not surprise you with any additional costs.

How can I get fixed teeth in one day? I thought the implants had to heal?

That is the beauty and unique nature of this procedure! Our state of the art 3D scan that acquires 3d images of your bone in your upper and lower jaw is used to determine where the most dense bone is in your jaws. This information is used by your Implant Dentist to place the implants with high stability. With a minimum of four implants being placed in high density bone, and splinting them together, we are able to connect the teeth to the implants the very same day. Splinting actually adds stabilization during the healing process (as long as your maintain a diet of soft food while healing 😁) 

What if another dentist has said that you are not a candidate or do not have enough bone for dental implants?

The majority of patients that have been told they are not a candidates for dental implants are actually good candidates for the All-On-4 Teeth in a Day procedure. A combination of using angled implants and using the front of the jaw bone where the bone is more dense allows us to complete this procedure for many patients that thought it was not an option. We can usually perform this procedure without the need for expensive grafting that delays treatment by months or years. What do the new implant bridges feel like?

The temporaries can feel a little bulkier, just due to using a temporary material that is weaker than the final zirconia. Once the final zirconia is developed custom to your mouth, they should feel as if they are very natural. Like everything, anything new takes a little time to get used to.

Is there excessive pain involved in this procedure?

Everyone is different, but for the majority of people the post operative discomfort is very manageable. We have a Pain Management Protocol that we employ with every patient to minimize post operative discomfort and to get you back to everyday life. Of course, during the procedure with sedation and anesthetic, you won’t have any pain and won’t be awake for any part of the surgery.

What is the recovery time like for this procedure?

For the vast majority of people, the recovery time is minimal. We recommend planning to have some discomfort, possible swelling, and possible bruising for 4-7 days following the procedure.

Can the extractions, implants, and fixed teeth really be completed in the same day?

Yes! This is the best way to heal and is the goal and expectation for every patient. Ultimately the stability of your bone is what determines if we can load the new teeth. With our state of the art 3D scanner as well as our premier dental implants that are designed specifically for stabilization, we can idealize the placement of implants into stable bone. 

Additionally, we complete a full face smile design so that your new teeth are as ideal as possible! Even though the first set of teeth you will receive are temporary teeth that we can improve on during the restorative phase, we do everything to make them a dream smile!

What credentials will my Doctor have for placing and restoring implants?

Dr. Loftus is a General Dentist that focuses on Implant Dentistry. This combination of restorative training and extensive implant surgical training is a great combination of the restorative/surgical combination that implant dentistry demands. With his focused training on full mouth Dental implants, he is employing cutting edge techniques, materials, and technology that allow for ideal placement of implants, ideal esthetics, fast healing times, and minimization of complications.

Do you offer warranties or guarantees?

We expect this treatment to last a lifetime, but we know that life can happen. Complications are rare, but when they happen, the majority happen during the first healing stage. We guarantee that we will get you to your final set of teeth, regardless of if a revision is needed during the healing time. 

Once you are in your final teeth, your bone and gums have healed and the only change we could expect is from a change in your overall health (like a bone disease) or if there is a complete lack of maintenance of your teeth and gums. All that to say, we expect issues and complications to be extremely rare. For when life happens, we work with Dental Warranty Co. that warranties your teeth and implants nationwide. If you move to Florida, they would be able to work with any dental office to correct an issue for 6 years after your initial treatment.

What about mini dental implants? Or snap in dentures?

Mini implants are not the same category of treatment as All-on-4. It is the general consensus of the Dental implant community that mini implants should only be used in a limited and temporary capacity. This is due to their high failure rate and limited application. You should strongly consider a second opinion if you are given a treatment plan for mini implants as a final implant treatment. We have corrected a large number of mini implant cases. 

Snap-in implant dentures are a good option for some patients. They are different than all-on-4 implant dentures because they come in and out of the mouth and function closer to dentures than they do to your natural teeth (all-on-4 implant dentures function at almost a 100% capacity compared to your natural teeth).

How long does the procedure take?

Only a couple of hours! If we start in the morning, for a single upper or lower arch patients are usually done around lunch time.

How often should I have my new teeth professionally cleaned?

You should come in every 6 months to have your teeth/implants checked and cleaned. This will help ensure the longevity of your new teeth and implants.

Can implants fail?

While titanium dental implants do have a 95+% success rate, yes, they can still fail. The vast majority of cases where the bone fails to heal around implants happens during the first couple of months (during the healing period). If your bone does not heal around the implants during that time, it will be corrected during the healing period but may extend your healing time. After integration of the implants into your bone, the only thing that can cause implant failure is a change to your systemic health that affects the bone, or bacteria that invades your gums (through lack of maintenance. Risk factors that increase your risk of failure:

  • smoking
  • Diabetes

How will you design my new smile?

Everyone has a unique smile and unique facial features makes every case different. The preparation for your new smile is very specific and systematic. We will essentially take a series of digital scans and photographs so that we can recreate your face, smile, and facial bones digitally. They design the ideal smile to match your facial structure. Once we develop the final design, we will have you try it in and make sure it matches what you have always wanted out of a smile.

Are All-On-4 bridges always attached to the implants the day of the procedure?

Almost always! There are a select few patients who have extremely poor bone quality which is not stable. In those rare circumstances the implants would not be attached until the bone has fully healed but would leave the day of surgery with a denture that sits on top of the gums.

Do All-On-4 teeth come in and out?

No, they are rigidly attached to your implants, which are firmly anchored in your jaw.

Can my permanent teeth be damaged?

While it is possible for zirconia and nano-ceramic to be damaged, it’s extremely uncommon. Zirconia is pretty much the strongest thing we can put in the mouth.

I already have an implant (or implants), can they be used for All-On-4?

If they have been placed in the correct location and have health bone and gums around them, then we can absolutely use them. But, if they aren’t we would plan on removing them to allow for ideal placement of a new implant. This won’t change your cost or healing times.

How long after my procedure should I wait to use a water flosser/Waterpik?

Patients should wait 4 weeks after their procedure before they begin cleaning with a water flosser.

I feel like I’m too young for this procedure?

We are always steering people towards keeping their teeth if possible. If there is a way you can have the smile and functionality while keeping your natural teeth, we will always recommend that route. But, not everyone is able to keep their teeth, even at a very young age. Our mission is to get as many people the smile of their dreams as possible. We meet every patient where they are, and work to get them healthy and happy with their smile!

Are my All-On-4 bridges removable?

No, you can not remove these yourself. Only the dentist is able to remove them. They are secured to your implants.

Can food get under my All-On-4 bridge(s)?

Every prosthetic should be designed to be cleansible and convex so that it is really difficult to get anything under the bridge. Additionally, it is made flush to your gums. So it should be very rare that anything get’s under it. You should ask your All-on-4 surgeon to show you an example of what the underside of the all-on-4 teeth look like. If they can’t show you a pristine convex design, you should get a second opinion.

What items should I have ready for after my procedure?

How long should I take off work for recovery after my procedure?

Of all full arch replacement options, All-On-4 tends to have the shortest time required before returning to work. Generally 4-7 days is sufficient. With that said, since there is always the chance of bruising and swelling, and you’ll need to get used to eating and speaking with your new teeth, the longer you can take off, the better.
